If you happen to stumble across any missing attachments - be it images or other kinds of files which generate an error when you try to download them - be sure to let us know and give us a direct link to the topic/post in question. This is an on going problem related to when we upgraded from IPB1.3 -> IPB2.0, and cleared out redundant content. It's an IPB problem though, with either the upgrade script or the AdminCP function which is meant to delete files that are no longer in use by any posts, as we never directly edited/deleted files. That's the way the cookie crumbles, I guess. I've restored a few broken files from an old backup, including Angnix's excellent Do's and Don'ts of sprites.

It means that your uploads directory is not properly CHMODed to 777, and it really has nothing to do with this topic, so read your FTP program's help file to find out how to CHMOD.
What is it that makes a complete stranger dive into an icy river to save a solid gold baby? Maybe we'll never know.

Depends on your FTP program. You know what FTP is though, right? You might want to take a look at IPB's installation guide too.
